Effective 5/1/2011 fuel surcharge is 26.0%

Fuel Surcharge Application: No fuel surcharge will be assessed if the Department of Energy's (DOE) Energy Information Administration Index indicates the average cost of kerosene jet fuel nationwide being $.70/gallon or below. If the DOE index indicates the average nationwide kerosene jet fuel cost exceeds $.70/gallon, the additional fuel surcharge will be assessed as follows:

If the DOE kerosene jet fuel index exceeds $2.90/gallon, an additional .5%(five tenths of one percent) will be added to the 22.0% fuel surcharge rate in increments of $.05/gallon. Fuel surcharge amounts are in addition to the published base door-to-door rate, and will be indicated as a separate line item on the billing invoice.

Notes: Changes in the fuel surcharge amount will be effective on the first business day of each month, based on the last month's average as taken from the DOE weekly national fuel index (updates from the DOE occur each Monday, excluding legal holidays which fall on a Monday) Kerosene jet fuel indexes will be taken using the average of the New York Harbor, U.S. Gulf Coast and Los Angeles fuel costs as published by the DOE. Verification of these fuel index averages can be obtained through the DOE internet website of http://www.eia.doe.gov/dnav/pet/hist/LeafHandler.ashx?n=pet&s=eer_epjk_pf4_rgc_dpg&f=m
